In consideration of being allowed to participate in any way with Boxing Alberta’s sports program, activities, and events, I acknowledge and agree that:

1. I hereby acknowledge and confirm that my participation in the sports events and activities carries with it inherent and serious risks, including but not limited to property damage, bone fractures, slight and serious physical injuries, body sores, or death. I further agree that my participation in the sports events and activities is voluntary and I assume all risks and responsibilities arising from my participation.
2. I understand and consent to be bound by the rules and policies outlined in Boxing Alberta’s policies and procedures. In any case that Boxing Alberta gets hold of competent proof of my violation of its policies and procedures, negligently or intentionally, it has the right to expel and disqualify me from participating in the sports events and activities.
3. I discharge Boxing Alberta, with respect to any injury, illness, disability, death, and loss to person or property arising from my participation.
4. I promise not to sue and indemnify Boxing Alberta from any liabilities arising out of my participation.

I have read this Sports Waiver, fully understand its terms, and that, by signing it, I have given up substantial rights, and sign it freely and voluntarily.
