Boxing Alberta Concussion Awareness & Risk
I understand the inherent risks associated with the sport of boxing, including the risk of concussion. Symptoms of a concussion can include headache, ringing in the ears, blurry vision, nausea, vomiting, confusion/fogginess, or dizziness. I understand that if I am experiencing any of these symptoms after receiving a blow to the head or body, I must notify a coach or trainer immediately as there may be risk of a concussion.
To return to training, an evaluation from a medical professional with a negative diagnosis is required. If it is a positive diagnosis and you have a concussion, you must follow Boxing Canada’s Concussion Protocol before returning to play. At which point a medical professional deems you no longer have a concussion, you may return to training. Participating in boxing is at the athletes own risk and Alberta Amateur Boxing Association will not be held liable for any injuries sustained during the course of the activity.
I hereby acknowledge I have read this form and will follow the protocols of Boxing Alberta.
